Information collected

When you book an appointment and choose to come for a massage,

I currently collect and process the following information:

  • Personal identifiers and contacts (for example, name and contact details) so that I can contact you in relation to your booking/enquiry.

  • Consultation form covering medical and lifestyle information and history (completed prior to your massage), treatment records (these include findings during your treatment, client feedback and any aftercare advice). This is required by my insurance policy and is also to ensure I can offer an appropriate treatment and practice safely - NB these are required for treatment to go ahead.

  • When using the website, information about your devices (e.g. IP address, operating system and browser details) are collected. This information is used to provide a better website experience for you. 

It is a condition of my Insurance Policy that I take and retain client records for at least 7 years following the completion of the last occasion on which treatment was given. 

I may need to share your data with authorised legal, regulatory and insurance authorities, if required to defend Anu Holistic Therapy. This will be the professional membership bodies I am registered with and the insurance company I hold my professional indemnity insurance with. 

After 7 years following the last occasion on which treatment was given, I will destroy your records by shredding them. 

How and where do I hold your data?

All consultation forms and treatment records are stored securely in a locked filing cabinet in my home. Only I have access to them and will take all appropriate steps to protect the confidentiality, integrity, availability and authenticity of your data.

Payments

PPayment is taken in advance of your appointment via Stripe and/or bank transfer. 


If you make payment online, data necessary to process your payment may be collected, such as your payment instrument number (such as a credit card number), and the security code associated with your payment instrument. You have 

  • The right of access to your personal data;

  • The right to object to the processing of your personal data;

  • The right to restrict the processing of your personal data;

  • The right to rectification of your personal data;

  • The right to erasure of your personal data;

  • The right to data portability (to receive an electronic copy of your personal data);

In exercising your Individual Rights, you should understand that in some situations I may be unable to fully meet your request. For example, if you make a request for me to delete all your personal data, I may be required to retain some data for taxation, legal, regulatory and insurance purposes.

You should understand that when exercising your rights, a substantial public or vital interest may take precedence over any requests you make. In addition, where these interests apply, I am required by law to grant access to this data for law enforcement, legal and/or health related matters.
